Dying in Kirklees – What matters to you?
We think it’s essential that end-of-life care and bereavement support is driven by real experience. We want to make sure people in Kirklees receive the care and support they need and want and we can only do that by listening to you.
That’s why we are working with Kirklees Palliative Care Partnership and The Kirkwood to understand how bereaved people feel about the care their relative or friend received before they died, and about the information and support available for bereavement in Kirklees.

Making a difference
Already your views are making a difference and a Best Practice Steering Group has been created to make sure your voices are heard. The more feedback we hear the more we can help.
What will happen next?
Feedback will be used to make improvements to the Kirklees Care Charter which sets the standard for access to end-of-life care and support for bereavement in Kirklees. Our aim is to make sure the health and social care system looks after the bereaved.
We anticipate that 6-12 months after the closing date of this survey we will be able to share with you actions and changes that have had a positive impact and influence on how care is delivered to people who are dying in Kirklees and how support is provided to relatives or friends who may have been affected by someone’s death. We will treat your answers respectfully and sensitively. A report will be produced which will contain anonymised comments and data from this survey and we will take care not to indirectly identify anyone*. This survey applies if the person who died was aged 18 or over and you or they live(d) in Kirklees. All information is anonymous and will only be used to improve end-of-life care and bereavement support in Kirklees.
