START - Admissions Avoidance (Locala)

START (Short Term Assessment Response Team) is dedicated rapid response team of nurses, therapists and health care assistants who work with patients to prevent unnecessary admission to hospital, and when patients are ready to return home from a stay in hospital, the team will ensure the correct support is place to reduce the risk of re-admission. The team work in the community and have a base at Dewsbury & District Hospital A&E. They assess patients who have been admitted and establish if it's possible, with the correct support in place, for them to be sent home rather than be admitted to hospital. They also meet daily with the acute care of the elderly unit and frailty within hospital to support the discharge of patients home or to another care setting. At Huddersfield Royal Infirmary they work with the Frailty Team to support discharge and follow up patients in the community to reduce the risk of readmission and provide support to A&E to identify patients that can be maintained at home safely.
