Living in the community questionnaire

October 24, 2016

CHANGE are working on a project about coming out of Assessment and Treatment Units (inpatient hospitals) and moving back into the community.

CHANGE would like to find out what is important for people with learning disabilities and autism when they live in the community.

They have a questionnaire that people can fill in and email or post back to them here: living-in-the-community-questionnaire-fillable and if anyone would like to try, they would like videos of people sharing their ideas.

The questionnaire is attached and more information about the videos, to give you an idea CHANGE have their own video asking for help

It is very important to hear people’s views because the findings will be used in Transforming Care with the Department of Health. The information will be used to tell health and social care workers what is best for people when they live in the community and we will compile a video to use in training.

If you would like more information you can speak to Claire or Sarah at CHANGE on: 0113 2426619
