Living in the community questionnaire

CHANGE are working on a project about coming out of Assessment and Treatment Units (inpatient hospitals) and moving back into the community.

CHANGE would like to find out what is important for people with learning disabilities and autism when they live in the community.

They have a questionnaire that people can fill in and email or post back to them here: living-in-the-community-questionnaire-fillable and if anyone would like to try, they would like videos of people sharing their ideas.

It is very important to hear people’s views because the findings will be used in Transforming Care with the Department of Health. The information will be used to tell health and social care workers what is best for people when they live in the community and we will compile a video to use in training.
