*Posture and mobility service survey apology*

Opcare recently sent out a survey seeking views about their service. Please see their statement below:

“We have carried similar surveys in the past in order to highlight areas for improvement of the service using a similar format but unfortunately on this occasion due to human error the wrong database report was used, and we hadn’t removed those patients who had since passed away.

We are very sorry for the distress this may have caused to these people’s families, friends and carers, and would like to offer our sincere and unreserved apologies for this error.

We would also like to be clear that Opcare sent the survey on behalf of Healthwatch Kirklees, a consumer organisation who have been supporting us to understand the needs of our clients. Opcare take full responsibility for the errors in the address list and the distress that this has caused.

We would also like to reassure anyone affected that we have reviewed our processes and taken the necessary steps to ensure that this will not happen again.
