Share your hospital to social care discharge stories

Healthwatch Kirklees and Calderdale’s latest work is focused on people’s experience of discharge from hospital and the social care services that support them once they return home.

We are looking at the discharge process and the various ways people get care immediately after including:

• people’s experiences of home first or reablement when they are discharged from hospital back into their own home with social care support;
• people’s experiences of transition or intermediate care beds;
• People discharged with Age UK’s Home from Hospital service.

We are speaking to patients, family and carers as well as staff in services to understand how the discharge into care is working and have the support of all relevant NHS and social care services for our engagement work.
