Leaving hospital: experiences of those needing reablement support at home or intermediate care

Hospital discharge is a common topic in the feedback Healthwatch receives. Our data shows that some people struggle when they are discharged without enough support, which can slow their recovery and increase the risk of being readmitted. Healthwatch Kirklees and Healthwatch Calderdale wanted to learn about people’s experiences with the different hospital discharge pathways.
